Instructions:

  1. Fill out each field with the appropriate information:
    • Type of Risk: Enter the category or type of risk identified (e.g., "Slippery Floors").
    • Description of Risk: Provide a detailed description of the identified risk, including potential scenarios and consequences.
    • Group of Risk: Provide a detailed description of the group of people at risk, including potential visitors and their physical state.
    • Likelihood: Rate the likelihood of the identified risk occurring on a scale of 1 to 5, where 1 is least likely and 5 is most likely.
    • Severity: Rate the potential impact or severity of the identified risk on a scale of 1 to 5, where 1 is least severe and 5 is most severe.
    • Mitigation Plan: Outline specific steps or actions to mitigate or reduce the likelihood or severity of the identified risk.
  2. Take appropriate actions based on the assessment to address the identified risks.
  3. Implement mitigation plans, conduct follow-up inspections, and revise risk assessments as necessary to ensure ongoing safety and compliance.
